A few days before your appointment, you will receive a link to fill out your medical history form & waiver. This form must be completed and submitted no later than 24 hours before your appointment time so it can be thoroughly reviewed to help prevent any potential contraindicators causing complications for your treatment and your results. If your forms have not been completed in time, your appointment may need to be rescheduled. 

DO NOT CONSUME ALCOHOL FOR THE 24 HOURS BEFORE YOUR APPOINTMENT. IF YOU ARE ON ANY BLOOD THINNING MEDICATIONS, PLEASE SEEK YOUR DOCTOR'S APPROVAL TO PAUSE THE MEDICATION FOR 2 DAYS BEFORE YOUR APPOINTMENT. YOU CAN RESUME THE MEDICATION AFTER YOUR APPOINTMENT. 

  All permanent makeup services require a 50% non-refundable booking fee. This fee is applied to the service price upon completion of the service. Should you no-show for your appointment, your booking fee will be forfeited. If you communicate a need to reschedule within 24 hours of your scheduled appointment, you will be given 1 reschedule opportunity. A second cancellation within 24 hours of your scheduled appointment will result in forfeiture of the booking fee.  

Temporary beauty service or waxing service clients will be banned from booking in the future. Those who cancel within 24 hours of their schedule appointment will be given one reschedule opportunity. will be given 1 reschedule opportunity. A second cancellation within 24 hours of your scheduled appointment will result in being banned from future bookings.
